Master question. I tend to think that that was only part of Meg`s "story" and thus untrue. Just something to sow more doubt in Dean. I bet if Dean HAD shot Sam, the black Demon-smoke would have come out, formed a "Ha ha" in the air and disappeared. Seemed kinda like her plan.
